Free Printable First Day of School Questionnaire
Gain valuable insights into your child’s mind through their favorite interests and evolving personalities. Parents aren’t the only ones who could use this fun activity. Educators love to include them in their back-to-school classroom activities. These printable questionnaires are a great way for teachers to get to know their new students during the first week of school. This worksheet doesn’t require much time to finish.
This back-to-school printable includes the following sections to be completed and referenced for years to come!
About Me
The first thing to fill out is your child’s name at the top of the upper right-hand corner. Then, move to the “all about me” section of the worksheet. This section covers basic information.
- Your child’s grade level
- Age
- School Name
- Teacher Name
- When I grow up I want to be – you may be surprised how often this answer changes with each passing school year.
My Favorites
This is all about your child’s favorite things including their favorite color and food items.
- Food
- Color
- TV Show
- Movie
- Hobby
- Sport
- Restaurant
- School subject
- Best Friends
- Add a favorite book in this section
This is My Smile
This is where you tape your adorable first day of school photo of your child. Hopefully, they are still in the stage where they enjoy taking those photos. If not, taking a photo of them can be just as fun to see how annoyed they get. I love how this Senior made the best of his school photos. Don’t have a First Day of School photo? No problem, take any photo of your child within the time frame or have them draw themselves. There are no rules here! Just have fun with it!
My Strengths
My favorite and most surprising answers of hers had to be in the “My Strengths” portion of the printable. I was really curious to know how she described herself and knowing what she wants to learn this year made my heart happy.
- 3 words that describe me
- Something I want to learn this year
How to use the First Day of School All About Me Printable
It’s fairly simple to document your child’s answers year to year. Consider saving them in a binder and referring back to their answers. It will be fun (and bittersweet) to see how they change throughout their school years.
- First, download by entering your email in our subscribe box below
- For best printing results, print in color on white regular paper or card stock.
- Take out a pencil from your new school supplies and start writing down answers.
Next, take the First Day of School photo and glue it on the questionnaire. Sometime that day (or maybe before or after, let’s be real) sit down with your child and have them fill out the questionnaire or read it to them aloud and help write out the answers if they are in lower grade levels. I asked my daughter the questions, and she answered, and then I helped her spell out the words.
They make a great keepsake that you can store in your Memory Box. It’s so rewarding to look back on their answers from the past answers.
BONUS: When you subscribe, you will also receive the LAST DAY OF SCHOOL QUESTIONNAIRE!
