This page includes space to tell your sitter where important things are and what emergency phone numbers to call in case an emergency occurs.

- Home Information: Include home address, phone number, cell phone number, and the locations for the gas valve, water valve, breaker panel, and first aid kit.
- Notes section: Include miscellaneous information including your kids’ bedtime routine, if any of the kids have food allergies.
Family Information

- Parent’s contact information
- Where parents will be and when they will return

- Update frequently.
- Give copies of these forms to various family members and your daycare provider (if applicable).
